This is a lightweight Addon Mod for More Injuries. Similar to Death Rattle, it mainly prevents pawns from instantly dying like in vanilla — or merely receiving a simple injury — when critical organs such as the heart, lungs, kidneys, or liver are missing or severely damaged. Instead, they will enter a More Injuries-style pathological process.
If you enjoy more detailed, brutal, and process-driven injury systems, as well as more realistic multi-organ failure scenarios, this mod should suit your playthrough well.
Main Features
- Structural Heart Failure
When the heart is missing, completely destroyed, or structurally damaged to a severe degree, the pawn will enter either “Clinical Death: Missing Heart” or “Clinical Death: Traumatic Cardiac Arrest.”
These are not ordinary arrhythmias. CPR and defibrillation cannot solve the underlying problem; the heart must be repaired or replaced. - Respiratory Failure
When total lung function becomes too low, the pawn will enter respiratory failure.
Respiratory failure will gradually cause systemic hypoxia, cerebral hypoxia, and acidosis, and may eventually lead to More Injuries’ original CardiacArrest in the terminal stage. - Acute Renal Failure
When total kidney function becomes too low, the pawn will enter acute renal failure.
Renal failure will drive uremia and acidosis, and in severe stages may lead to terminal cardiac arrest. - Acute Liver Failure
When liver function becomes too low, the pawn will enter acute liver failure.
Liver failure may cause coagulopathy, acidosis, hepatic encephalopathy, and in severe cases may progress to coma and terminal circulatory collapse. - These Conditions Can Trigger Each Other
More Injuries itself already includes coagulopathy and hypoxia systems that can damage different body parts. When critical organs are reduced to very low health by these conditions, the corresponding organ failure symptoms may appear and accelerate the dying process. In the late stage, sometimes all you can do is provide palliative care.

It is strongly recommended to also use No More Lethal Damage Threshold, and place it after this mod.
Compatibility
This mod does not modify More Injuries’ original files and does not override vanilla Defs.
It mainly implements the bridge logic through Harmony patches and new Hediffs.
In theory, it should be compatible with most medical, bionic, and race mods.
However, problems may occur if another mod heavily modifies death checks, pawn capacities, life stages, or body structures.
Special notes:
- Race mods with custom body structures may not trigger the corresponding mechanisms if they do not use standard Heart / Lung / Kidney / Liver body parts.
- Obviously, due to overlapping functionality, this mod is not compatible with Death Rattle.
- Archites and ghouls have not been tested, but they should not cause severe bugs.
- Regardless of how you configure More Injuries, this mod will not apply its related mechanisms to shamblers.
Q&A
Q: Will this mod make pawns immortal?
A: No. It only changes some critical organ failures from “instant death” into “pathological progression.” Brain destruction, torso destruction, and severe downstream complications can still kill pawns.
Q: If the heart is gone, can CPR or defibrillation bring the pawn back?
A: No. A missing or structurally destroyed heart is not ordinary cardiac arrest. The heart must be repaired or replaced.
Q: Can cardiac arrest caused by respiratory failure be treated with CPR or defibrillation?
A: Yes. Respiratory failure can cause More Injuries’ original CardiacArrest in the terminal stage, so it can interact with More Injuries’ CPR / Defibrillator mechanics.
Q: Are dialysis or ICU devices currently included?
A: Not yet. I’m lazy.
Q: Can this mod be added mid-save?
A: In theory, yes. However, since this mod involves health conditions and death checks, backing up your save first is still recommended. If you want to uninstall it, make sure that no pawns on the current map or pawns that have left the map have any related hediffs.
